Job Summary

This position is responsible for assisting individuals enrolled in the Respite program in obtaining and maintaining permanent, stable housing. This position is also responsible for care coordination with partner agencies involved with households and assisting them in moving in a planned manner towards achieving personal independence.

 

Job Duties/Responsibilities

  1. Utilizing the principles of housing first and progressive engagement, provide case management to Respite clients and develop exit plans to stable, permanent housing.

     

  2. Develop client centered housing stability plans for a caseload and work closely with those individuals to facilitate the movement into and maintenance of permanent, stable housing.

     

  3. Provide support services according to professional ethical conduct guidelines.

  4. Secure appropriate services for HOC Respite clients including mental, physical health and/or other needed services and make referrals when appropriate.

     

  5. Maintain a current knowledge of community resources available to clients; network with organizations that provide housing and other services applicable to Respite clients.
  6. Monitor Respite program policies, procedures and protocols to ensure compliance with applicable laws, regulations and guidelines including Fair Housing, Washington Landlord-Tenant Act, and Department of Housing and Urban Development.
  7. Act as landlord liaison during housing search. Facilitate background check screenings, application submissions, appeals when necessary, and inspection and lease signing appointments.

     

  8. Link recipients with primary care services and health homes; substance abuse treatment providers, vocational, education, employment, volunteer and social supports.

     

  9. Provide assistance in independent living skill-building, including financial and life-skills coaching.

     

  10. Maintain client files according to funder requirements. Ensure client data in HMIS is current, accurate and entered in real time.

     

  11. Complete monthly reviews of data relevant to the project and provide updates to direct supervisor.

     

  12. Meet weekly with fellow House of Charity case managers to discuss and review caseload and progress on client centered goals.

     

  13. Maintain training to ensure up to date qualifications in best practices such as progressive engagement, motivational interviewing, mental health first aid and others as directed by supervisor.

     

  14. Continually seek the improvement in the quality of the HOC operations and case management program.

     

  15. Maintain appropriate professional boundaries with clients and co-workers. Adhere to an appropriate self-care plan.

     

  16. Perform other relevant duties assigned by supervisor.

     

  17. Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.

     

  18. Perform related functions necessary to support the mission and core values of Catholic Charities.

     

Job Qualifications

To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.

 

Education/Experience: to perform this job successfully, an individual must have a Bachelor’s Degree in Social Services or related field and one year of relevant experience in direct client services; OR an AA Degree with 3 years of relevant experience in direct client services; OR 5 years of relevant experience in direct client services.

 

Successful candidates will demonstrate in their cover letter how they meet the minimum education and/or experience qualifications.

 

Certificates/Licenses: To perform this job successfully, an individual must have a valid Driver’s License and successfully pass background check applicable to position.

 

Physical Abilities: To perform this job successfully, an individual must be able to:

  • Regularly sit, stand, climb, walk, hear/listen, talk

  • Frequently lift up to 25 pounds, pull/push, carry, grasp, reach

  • Occasionally crawl, stoop, kneel

Workplace Environmental Conditions

While performing the essential duties/responsibilities of this job, the employee will be:

  • Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the daily activities.

  • Client Population may include persons involved in gangs, with substance use concerns, homelessness and/or tendencies towards violent behavior.
