Job Summary
This position is responsible to assist clients at Eastern State Hospital (ESH) with transitioning from ESH to independent living in permanent housing and improving emotional and mental stability through care coordination with shared community partners.

Job Duties/Responsibilities

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties/responsibilities of this job.

  1. Receive referrals from DSHS for Eastern State Hospital (ESH) patients ready to discharge into a community living setting. Must be able to handle multiple referrals at any given time, including phone calls; secure email referrals, and coordination of entry into the program. Coordinate with DSHS and ESH to accept referrals deemed appropriate for SMS housing services.
  2. Provide comprehensive housing search and placement services and care coordination for ESH clients by partnering with ESH Social Workers, DSHS/Aging and Long Term Care, landlords, and community-based Behavioral Health organizations that provide services to shared clients, including working as a liaison between said entities.
  3. Secure an appropriate, short term (2 year) client-identified housing option for ESH clients. Assist ESH clients with applying for three long-term/permanent housing options.
  4. Maintain a comprehensive list of community resources available; network with organizations that provide housing, mental and physical health and/or other needed services and make referrals when appropriate.
  5. Provide move-in assistance, which consists of coordinating purchase of move-in furniture/household items, deposit payments, and initial food. Submit appropriate requests for reimbursement for said expenses from ALTSA.
  6. Once ESH clients are housed, monitor and act as a liaison to ensure compliance with leasing obligations, Washington Landlord-Tenant Act, and Department of Housing and Urban Development, if appropriate. Submit appropriate documentation to ensure rent is paid by DSHS for client’s short-term housing unit.
  7. Maintain client case files and other documentation in accordance with program and COA standards, contractual requirements and community best practices. Complete data forms for case files as required.
  8. Coordinate 24 hour on-call services with SMS Shelter for ESH clients during days off.
  9. Work in collaboration with the rest of the SMS team for resource sharing and group partnership.
  10. Track billable hours according to DSHS time cards and Provider One systems, and CC payroll software.
  11. Perform other relevant duties assigned by supervisor.
  12. Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.
  13. Perform related functions necessary to support the mission and core values of Catholic Charities.

Job Qualifications

To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.

Education/Experience: to perform this job successfully, an individual must have a Bachelor’s Degree in Social Services or related field. Relevant experience in direct client services. Experience in medical care transitions preferred.

 

Certificates/Licenses: To perform this job successfully, an individual must have a valid Driver’s License and ability to drive for work use. Successfully pass background check applicable to position. Must obtain and maintain CPR and 1stAide certifications.

Physical Abilities: To perform this job successfully, an individual must be able to:

  • Regularly sit, stand, climb, walk, hear/listen, talk
  • Frequently lift up to 25 pounds, pull/push, carry, grasp, reach
  • Occasionally crawl, stoop, kneel
  • Clearly see 20+ feet, with or without corrective lenses, ability to focus

While performing the essential duties/responsibilities of this job, the employee will be:

  • Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the daily activities.
  • Client Population may include persons involved in gangs, with substance abuse concerns, homelessness and/or tendencies towards violent behavior.
